Palliative Care Advanced Practice Nurse

Palliative Care Advanced Practice Nurse Job Title: Community Palliative Care Advanced Practice Nurse Reports To: Palliative Care APN Manager Purpose of the Position : Provide palliative care for person with serious, complex and/or life-threatening conditions and their families.

Participate in comprehensive medical and psychosocial evaluation of patients and families, skilled and meticulous symptom management, and discharge planning and continuity of care.

Serve as clinical and educational role model and mentor.

Provide care and support in collaboration with the primary care team Ensure compliance with practice standards and codes.

Job Responsibilities: Conduct comprehensive, focused assessments of patients with palliative care needs.

May provide care for hospice patients and partner with hospice teams.

Formulate, implement, monitor, evaluate and modify palliative care plans with input from palliative care team, attending physician and other collaborating care providers.

Maintains communication with attending physician of (and patients primary care physician if not managing inpatient care) family discussions, goals and treatment plans.

Communicates with collaborating physician per standard regarding patient care as needed.

Work with patients and families to determine their understanding of proposed treatment plan and treatment options.

Assist patients and families to develop self-care capabilities relative to identified problems Assist in all facets of care coordination for palliative care referrals.

Maintain accurate recording for palliative care data base.

Engage in quality improvement in patient care services and department functions, including evaluating the effectiveness of the health care delivery system in meeting the health care need of palliative care patients and families.

Advocate for improvements in palliative care through participation in professional, private and governmental groups.

Provide clinical expertise and guidance for undergraduate and/or graduate nursing students in the specialty area.

Act as resource person for clinical information and education as requested by community agencies.

Plan clinical and education interventions which are consistent with individual/organizational needs and goals, and evaluate change/outcomes.

Implement patient and family education in collaboration with the interdisciplinary team.

Participate in palliative care research, including quality and performance improvement initiatives.

Define goals for professional growth, and participate actively in professional growth and longevity.

Perform other duties as assigned Job Qualifications Unrestricted license to practice in the state.

Minimum of 5 years nursing experience, including care of patients with serious, complex and /or life-threatening conditions, and those at end of life; clinical nursing in oncology, hospice, intensive care preferred.

Minimum of two years of experience as an APN in a related field strongly preferred.

Masters Degree with certification as Advanced Practice Nurse.

Hospice and Palliative Care Certification or eligibility desirable, ACHPN.

Knowledge of medications and prescriptive authority per state law and Nurse Practice Act.

Experience with end of life care, including advance care planning, pain and symptom control of the terminally ill, and bereavement care desired.
