In this position, you may: Provide geriatric and adult primary care to patients in your geographic area.
Youll visit patients in their homes both in senior communities and their private residences where our patients reside to conduct the visits.
Have autonomy in clinical decision-making with the back-up support of seasoned practitioners and administrative team.
Duties and Responsibilities may include: Managing a primary care caseload of geriatric/internal medicine patients Managing comorbid conditions such as diabetes, congestive heart failure and behavioral disorders Issuing appropriate orders, prescription refills and completing other administrative tasks post-visit Utilizing a comprehensive EHR to manage patient care.
Collaborating with a remote medical assistant to manage caseload and address emergent issues and questions (M-F, 8am-5pm) Communicating effectively and promptly with team members, patients, caregivers and families via phone, email and fax Maintaining accurate and complete documentation of all clinical information and updating patient records in a timely manner Traveling to Crown Health offices as required to complete administrative tasks and attend staff meetings and events Required Qualifications: Physicians and Mid Level practitioners require completed degrees and certifications along with Masters/Doctoral degree from an accredited Nurse Practitioner or accredited Physician Assistant program when applicable.
