Nurse Navigator – Urology

GWU Medical Faculty Associates

This is a Full-time position in Washington, DC posted October 2, 2021.

Position Summary

Under general administrative direction for Urologic Oncology Program, coordinates referrals from health care facilities and individual providers and provides a significant level of patient education related to illness and planned management and treatment.  The Nurse Navigator will triage indicating needs taking primary responsibility for logistical concerns. The Nurse Navigator will also act as the community liaison for support programming to ensure that the patients are aware of the different services offered.

We are looking for 2 Nurse Navigators for two different locations: 

  • Urology (Urologic Oncology) – 2150 Pennsylvania Ave NW

  • Urology – 2300 M St.NW

Essential Duties and Responsibilities 

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.  Other duties may be assigned.

  • Liaison for Urologic Oncology patients in need of multiple services and support

  • Provide education and clinical triage/troubleshooting of patient questions via proactive communication pre- and post-procedures/surgery

  • Conduct 24-48hr post-procedure/post-operative patient checks to screen/preempt incipient complications

  • Provides navigation services by facilitating resources and processing applications for various patient assistance programs, including but not limited to financial, Medicaid enrollment, free or reduced-cost medication, social security benefits, disability benefits, copayment assistance and insurance coverage, medical leave/FMLA paperwork, as well as adjustment counseling, social work needs and access to patient support groups.

  • Coordinate patient feedback and solicit avenues for care delivery improvement/optimization

  • Synthesize provider recommendations from clinic notes into a clear, concise “Plan of Action” for urologic oncology patients in need of multi-component staging workup, consultations, and multi-modal treatments

  • Provide patient literature/brochures/access to support groups

  • Coordinate complex multi-disciplinary care with Medical Oncology and Radiation Oncology

  • Pre-screen new Urologic Oncology patients to ensure records are comprehensive and complete (i.e. referring provider notes, Oncologic history notes, imaging reports, and disks, pathology reports, outside hospital pathology slides transferred to GW for pathologic re-review); write a summary note of relevant information/oncology history in EMR prior to patient visit

  • Clinic prep notes for returning patients which include scanning notes/imaging, contacting referring physicians or imaging group, and contacting patients on any issues on items needed for visit. Ensuring that items discussed at prior visit are completed prior to upcoming visit.

  • Alerts provider to urgent situations that cannot wait until scheduled follow up, and triage patients for an urgent clinic visit with clinic Physician Assistant,

  • Work with all members of the surgical and oncological team to seamlessly coordinate the care delivered and necessary support for the patient and their families.

  • Collect and submit documentation required for ancillary oncologic testing (i.e. germline testing, tumor genomic testing,) and specialty medications

  • Coordinate with clinic RNs for medication prior authorizations of specialty medications, and scheduling clinic RN procedures (i.e. intravesical chemotherapy/immunotherapy infusions, hormonal injection appointments)

  • In collaboration with patient’s care team, provides long-term education and references to resources, psychosocial support, and advocacy after surgery resulting in optimal patient outcomes

  • Handout and review OR prep packet with the patient.

  • Arrange/coordinate education, medication, and scheduling preparation for office-based procedures

  • Assist providers in addressing insufficiencies in care provision by coordinating follow-up, orders, and referrals.

  • Chart review to ensure completeness of records for new patients seen in the Genitourinary Multi-Disciplinary Clinic; coordinate next steps in care and collaborate with Oncology Patient Navigator

  • Liaise with clinical trial coordinators to facilitate enrollment of new patients in clinical trials and to coordinate care, tests, and next steps for patients already enrolled

  • Arrange Peer to Peer discussions with Physician assistants as necessary

  • Sign off on tasks, patient queries, patient laboratory/imaging results from the clinic/monitor FMH inbox; ensure that follow-up appointments are already scheduled to review time-sensitive results such as imaging scans and biopsy/pathology results

  • Coordinate referrals from other providers (both within and outside MFA)

  • Coordinate referrals for procedures from other Urology staff (i.e. prostate fusion biopsy, pre-radiation fiducial markers & SpaceOAR hydrogel placement)

  • Arrange educational/information sessions with clinic RNs and clinic MAs to ensure compliance with evolving oncology protocols and clinical trial requirements

  • Be proactive about identifying strategies for optimization/improvement of clinic-based care pathways with a focus on patient education and time-efficiency

Qualifications

Education       

  • Bachelor in Nursing. 
  • Current D.C. Licensure as a registered nurse or evidence of application for licensure in D.C.

Experience      

  • Two years of experience as a registered nurse is required.
  • Experience in public health or health/patient education preferred.
