The Baylor Scott & White Health Nurse Navigator utilizes the nursing process of assessment, planning, implementation, and evaluation of patient according to the Physician orders and establishes a plan of care.
Our Nurse Navigators coordinates with physician’s office the testing to be performed in order to make a definitive diagnosis, communicates with all members of healthcare team, and facilitates timely access to quality medical care from diagnosis through survivorship. In addition, the Nurse Navigator collaborates with physicians and other health care providers to improve patient care within an interdisciplinary model; educates patient and family members on diagnosis, treatment options and available services; and provides direction and support to the patient/family and serves as patient.
The ideal Nurse Navigator functions autonomously and in collaboration with other members of the healthcare team as a patient advocate, educator and coordinator, and possesses strong verbal and written communication skills.

QUALIFICATIONS
- EDUCATION – Bachelor’s
- EXPERIENCE – A minimum of 3 years experience required
- CERTIFICATION/LICENSE/REGISTRATION –
Registered Nurse (RN)
Basic Life Support (BLS): BLS in 30 days of hire or transfer.
