This is a Full-time position in Chicago, IL posted February 15, 2021.

The Nurse is part of a team that provides mental health services directly to our members; within that team, the nurse will assess patients, administer medication, and provide education for the patient.

ESSENTIAL DUTIES & RESPONSIBILITIES

  • Visits members at their homes or in the community to assess medical and psychosocial needs. Provides services according to nursing standards and monitors clinical status and response to treatment.
  • Provides consultation to clinical team regarding members’ medical issues. Serves as a resource for the clinical team on general medical issues.
  • Works closely with all consulting psychiatrists and medical doctors as assigned. This may include but is not limited to: medication administration, scheduling appointments and coordinating care as indicated by the members’ service plans.
  • Provides education about available resources or options available to the members and assist in the application process.
  • Advocates and assists with problem solving to help the client access support from their community and/or family.
  • Works cooperatively with inpatient hospital staff, keeping logs and other records of all contacts as required by agency practice. Performs and participates in crisis intervention when necessary.
  • May be required to be on call for emergency response coverage.

EDUCATION/CERTIFICATION

  • Bachelor’s degree from an accredited school of nursing required. Master’s degree preferred.
  • Licensed Registered Nurse required.

REQUIREMENTS

  • Daily access to a personal vehicle.
  • Current and valid driver’s license.
  • Willingness to provide transportation to members of the program.
  • Must obtain and maintain liability car insurance of:
    • $100,000 per person bodily injury
    • $300,000 per accident
    • $100,000 property damage
  • Experience working with persons diagnosed with mental illness preferred.
  • Computer skills required, including knowledge of database functions and maintenance
