The MSW Social Worker provides psychosocial services, including patient/family counseling, crisis intervention during hospitalization, comprehensive discharge planning, coordination of community resources and assisting patients and families in the decision making process. Provide Discharge Planning.Work together with other members of the multidisciplinary treatment team to provide consultations.Provide staff development and training on assessments, reassessments, care plan development, interventions, prioritizing needs, case conferencing and follow-up.Conducts Psycho/Social problem assessments.Performs other duties as needed or assigned.
