If you have the passionto go the extra mile to care for seniors in need of AffordableAssisted Living services, come join our Solid Team.Responsible forconducting assigned health assessments, developing service plans, performinghealth monitoring and personal care programs of the assisted living community.Clarifies initial or hospitaldischarge medication orders and contacts the Physician to review the resident’sstatus and medication plans.Develops assigned service plansfor residents based upon the resident assessment(s).
The service plan should bedesigned to meet the personal care needs of the resident and to monitorsignificant health conditions, while also promoting resident independence anddignityPromotes resident empowermentand self-care activities including self-medication.Performs assigned quarterly andannual reassessments on residents often times utilizing computer and softwareprogram.
Also performs such assigned reassessments when the resident’s healthcondition warrants.
Must be a Licensed LPNwith excellent verbal and written communication skills.
Prior LPN senior careexperience desired.
