As a Licensed Practical Nurse (LPN), you will work with referral sources from the healthcare community to plan and direct services to meet the needs of individuals and families in their homes. You will be responsible for completing a physical assessment and determine any patient home care needs, assist in developing and implementing that plan of care for each patient while using VNA of Floridas case management model.
Licensed Practical Nurse (LPN) Requirements:
Valid driver”s license for the agency state of operation or surrounding area with appropriate auto insurance
Current Licensed Practical Nurse in the State of Florida
Preferred 1 year of nursing experience
Home Health and/or hospital experience a plus
