We have an immediate opening for a Home Health Licensed Practical Nurse (LPN) in Fountain Inn, SC to provide effective and efficient in-home patient care under the supervision of a registered nurse.

Who you are: Graduate of a Licensed Practical Nurse (LPN) program and has a current license as a LPN/LVN in the South Carolina.

At least one-year or 1200 hours of experience as a Licensed Practical Nurse (LPN).

Participates in the development and evaluation of the plan of care in partnership with the patient, representative (if any), and caregiver(s).

Provides services that are ordered by the physician as indicated in the plan of care.

Recognizes and reports changes in the patient’s condition to the RN supervisor.

Assists the RN in carrying out the patient plan of care.
