Position Description The Hospice Contracted Physician is a hospice employee or is under contract with the Hospice who is a doctor of medicine or osteopathy, and is designated by the Hospice Care Program.
Essential Duties and Responsibilities Be responsive to the medical community to assure high quality, appropriate and efficient utilization of hospice services Assists in investigations of important problems in patient care and clinical performance Assists the Agency in the coordination of patient care between Medical Director, clinical staff and patients’ attending physicians Acts as the physician representative of the interdisciplinary group (IDG) and/or attending physician to certify and recertify for subsequent election periods that the patient is terminally ill Initial Certification of Terminal Illness Physician will review the clinical information for each Hospice Patient and provide written certification, as appropriate and in accordance with state and federal guidelines, that it is anticipated that the Hospice Patients life expectancy is 6 months or less if the illness runs its normal course Physician must consider the following when making this determination, and document his or her findings in the clinical record: The primary terminal condition; Related diagnosis(es), if any; Current subjective and objective medical findings; Current medication and treatment orders; and Information about the medical management of any of the Hospice Patients conditions unrelated to the terminal illness Follows agency policies and procedures to operate and furnish services in compliance with all applicable Federal, State, and local laws and regulations Other duties as assigned by management REQUIRED Knowledge, Skills and Experience Is a Doctor of Medicine or Osteopathy Legally authorized to practice medicine in the state of practice Must maintain a valid driver’s license and good driving record Has mental and physical health necessary to meet the demands of the position Preferred Knowledge, Skills and Experience Hospice Medical Director Certification 06022021
