Responsible for performing activities resulting in timely and accurate completion of the electronic medical record. Ensures accuracy, legibility and appropriate order/index of all scanned records.
Processes external and in-house requests for medical records in a timely manner. Under the direct supervision of the clinic manager. Employee is subject to call back and overtime as required by the clinic.
Maintains familiarity with all record storage/archive systems and any related processes involved with these functions.
Maintains all aspects of a recordkeeping system in a ‘normal course of business’ setting.
Performs duties accurately and efficiently according to departmental policies and procedures.
Maintains proficiency in medical record/information retrieval including but not limited to Power-chart, or other clinic EHR systems.
Preps, scans and validates into Power-Chart as appropriate, any paper or faxed records received.
Attaches records to outgoing order or referral as necessary for quality reporting/ Meaningful Use/ MIPS purposes.
Performs review of patient medical record documents requested for completeness and compliance prior to release of any records.
Manages incoming faxes from outside agencies, clinic, and physician offices and establishes priority for each.
Responds to all requests in order of priority.
Assures all work is done in a timely and efficient manner.
Works with copy service to assure seamless service for customers.
Troubleshoots equipment failures and corrects whenever possible.
Promptly notifies manager of equipment failures and system related problems.
Maintains current training on HIPAA and any regulatory changes in the handling of medical records.
Is able to handle high volume of divergent demands effectively through good organization, setting of priorities, timely decision making and stress management.
Relates well to other departments and to clinic staff and participates on multi-disciplinary teams as directed by management.
Assists in training cross training and coverage for co-workers in the medical records department and other clinic staff as directed by management.
Performs all related responsibilities in a positive and professional manner.
EDUCATION:
Knowledge of basic principles, concepts and techniques of medical records, normally acquired through 1-3 years of experience in Health Information Management.
Have the ability to read, write, and follow technical office procedures as acquired through high school diploma or equivalent.
LICENSURE, REGISTRATION, CERTIFICATION:
None required
EXPERIENCE:
Knowledge of computer applications (Microsoft Windows, computerized equipment, etc.) a must.
Keyboarding skills preferred.
Ability to navigate through the computer (software and hardware) and recognize potential issues. Ability to apply critical thinking skills to complex medical record policies.
Knowledge of hospital and departmental policies and procedures, State and Federal Laws pertaining to HIM, medical records forms, and medical terminology as acquired through at least 3 months of on the job training. One to two years of office experience preferably in Health Information Management or equivalent. Knowledge of office procedures, equipment, reports and clerical skills as normally acquired through 1-2 years office experience.
Experience with patient referrals from one office or clinic setting to another strongly preferred.
