Geriatric Nurse Practitioner

*Starting Line Wellness *is looking for a part time Nurse Practitioner(s) or Physician Assistant to preform primary care visits in Assisted Living Community in Hopkinton, MA and surrounding areas.

Starting Line Wellness is your full-service nurse practitioner practice bringing the care to our patients within an Assisted Living Community.*Core Competencies: *

· Ability to stay informed of current industry trends, to learn and apply new concepts; Ability to identify own areas of opportunity and set and monitor self-development goals.*Position Responsibilities: *
· Performs initial and periodic assessments of patients and appropriately identifies and documents problems issues in the medical record.
· Evaluates episodic illnesses and issues appropriate diagnostic and therapeutic orders..
· Performs diagnostic and therapeutic interventions as indicated.
· Implements plan of care in a safe and effective manner.* Prescribe medications* Inform patients about their health conditions and prescribed medications* Provides education and training to residents, caregivers and staff in relevant topics* Collaborates with physicians/specialists and other multi-disciplinary team members to manage episodic and chronic medical needs of the patients oversee patients’ overall health* Promote good health practices* Documents each patient encounter in a timely manner using the appropriate electronic medical record system* Advance professional knowledge and conduct research*Qualifications**Education: * Masters Degree required.

Graduate of an accredited Nurse Practitioner program.*License: * Licensed as nurse practitioner by the Commonwealth of Massachusetts Board of Registration in Nursing.*Certification: * National Certification as Geriatric, Adult or Family Nurse Practitioner.*Experience: *Four years of Nurse Practitioner experience preferred.Two years as nurse or nurse practitioner in geriatric facility preferred.Starting Line Wellness is an equal employment opportunity employer.
