Work Hours/Shift:
- Full-Time, Monday – Friday
You Will Be Responsible For:
- Monitor for alerts for ED registration through State of Florida Event Notification System
- Evaluate case for target conditions, provide support and discharge planning consultation to ED staff as needed and coordinate post-ED care and communication with primary care practice
- Perform real-time clinical reviews for appropriate next site of care using facility EMR. Contact and/or follow up with the respective ED clinician as needed.
- Provide ongoing updates as necessary depending on the patient condition. Update case status in EMR system.
- Maintain open communications and build robust collaborative relationships with ED providers, ED staff and primary care providers to work toward common goals of excellent patient care at the most appropriate level of service
- Ensure our patients are properly identified at the point of service throughout the care continuum.
- Assist ED staff with understanding and utilization of benefits enhancement as applicable
- As needed, on-site visits with collaborating emergency department staff and leadership
- As needed, outreach to members and beneficiaries post-discharge from emergency department
- As needed, on-site visit to beneficiary while in the emergency department
- As needed, facilitate, and coordinate next site of care discussions, arrangements and follow up. Next site of care can include home care, SNF, hospice, urgent care at home and /or referral to division health management
- Adheres to case management standards of practice for care management promoting quality care and cost-effective outcomes that enhance physical and psychosocial health of individuals
Qualifications
What You Will Need:
Education and Experience Required:
- Bachelor’s degree in nursing
- Active RN license for the state of Florida
- 3-5 years of acute care and/or emergency department experience
Licensure, Certification or Registration Required:
- Licensed Registered Nurse in the State of Florida (RN)
- Certified Case Manager (CCM) preferred
Knowledge and Skills Required:
- Knowledge and understanding of utilization management criteria
- Excellent analysis, problem- solving and critical thinking skills.
- Ability to use data to measure progress and achieve outcomes.
- Strong business communication skills to resolve issues and communicate with internal and external parties.
- Ability to work with little supervision
- Self-motivated
- Microsoft office suite of applications experience
- Excellent time management skills to effectively navigate in fast-paced work environment with time constraints in place
- Ability to interact effectively with providers and other health care professionals

Job Summary:
The ED Care Coordination nurse works under the direction from the PHSO Care Integration Manager and is responsible for coordinating care to ensure value-based lives presenting to the ED receive care in the most appropriate setting and to coordinate and educate patients and providers on services to support appropriate setting. (S)he will be in frequent communication with various ED staff, primary care providers and population health leadership. The ED Care Coordination nurse is responsible for admission/discharge/transfer monitoring and care coordination for value-based care patients in the emergency department. The ED Care Coordinator will maintain current knowledge of value best care best practices and CMS initiatives such as benefits enhancement for direct contracting entities.
