The Community Health Nurse – RN (CHN-RN) provides services to Hope support recipients through their work in providing evaluations, health care planning, management, and ongoing monitoring of individuals who experience developmental disabilities, including adults and children with complex medical conditions and others who experience mental health diagnoses.
The CHN-RN provides education to support recipients, families and staff.
This position works independently in both urban and very remote communities, and may be called on to respond to crisis situations.
The CHN-RN utilizes professional experience and knowledge to function independently, providing direction and education to staff and families.
The Community Health Nurse – RN maintains loyalty to the organization, its beliefs, values, mission, vision, and expectations, and to the recipients of services.
Essential Job Duties
1.
Evaluates and coordinates the widest range of medical and community resources and provides ongoing monitoring and health care management for individuals requesting supports.
2.
Collaborates and provides clinical support and guidance regarding health issues and concerns to families, staff and support recipients.
Provides functional authority over direct support professionals to maintain a high quality of care.
3.
Collaborates with Supervisors to monitor staff delivery of health supports and takes appropriate action to correct any deficiencies.
4.
Collaborates with support recipients and families to identify services needed and, where appropriate, request community referrals.
Coordinates services with various service providers.
5.
Facilitates communication and collaboration with parents/guardians, hospital staff, care coordinators, discharge planners, medical vendors, physicians, therapists, and others as needed to manage and/or advocate for the support recipient’s service needs.
6.
Oversees the development and implementation of health strategies in the person-centered support plan and health care plan, and identifies needs for revision.
7.
Provides the professional data necessary for the completion of health care coordination and funding documents.
8.
Evaluates current health status and health supports delivery.
Advises on necessary modifications by traveling to support recipient’s home and/or community regularly.
This may include homes in small towns and rural villages not within close proximity to formal health care.
9.
Develops and implements individualized training checklist for staff and families, using professional knowledge and gathered information.
Ensures the coordination of training with various agencies and community resources.
Evaluates the quality of support recipients health care supports and effectively communicates and advocates for necessary changes to enhance the quality of care.
10.
Documents health care information and other pertinent information in a timely and professional manner.

Job Requirements:
Education and Experience
1.
Bachelor’s degree in Nursing required.
2.
Two years of RN experience required.
Four years of experience preferred.
Experience working with individuals who experience intellectual disabilities and in community health preferred.
3.
Current State of Alaska Registered Nurse license required and must be maintained throughout employment.
Additional Requirements
1.
Possesses a valid Alaska driver’s license and has at least one year of driving experience (may include permit time).
Obtains a driving record at time of employment, annually thereafter, and as requested by management.
2.
Able to safely operate Agency vehicles when needed.
Possesses reliable transportation and vehicle insurance when transporting support recipients or conducting Agency business.
3.
Meets State and Agency requirements for passing a criminal background check upon hire and as required thereafter.
4.
Lifts 25 pounds from a squat and kneel position on the floor.
5.
May be required to frequently walk, bend, stoop, kneel, reach, pull, and/or push.
6.
Works at a desk/computer workstation for a minimum of six hours per day.
7.
Completes all required State, Agency, department, and position specific training as assigned.
8.
Obtains a tuberculin clearance at time of employment and annually thereafter.
