Description
The Clinical Nurse Navigator serves as the coordinator for the care of patients in their specific specialties.
The Navigator is responsible to provide support, education, referrals, coordination of care, and other assistance to the patient and their family prior, during, and following their healthcare experience.
The position is responsible for facilitating integrated care through communication with strategic members of the interdisciplinary team, collaborate to provide staff education and works closely with patients, families and members of the health care team to optimize patient satisfaction and achieve optimal outcomes.
The Navigator will coordinate efforts between physicians, healthcare administration, family, community resources and staff while tracking the progress of the patient throughout their continuum of care.
Required Certification in Related Specialty must be obtained within the first 6 months if not current.
Certifications and Licensures Required RN (Registered Nurse) State of Florida Required Nurse Specialist within 6 months
Education Required Bachelor’s Nursing Preferred Master’s Nursing
Experience Required 2 years Acute Care Or 2 years Related Field
