Job Description
We’re hiring a Licensed Independent Clinical Social Worker or Licensed Mental Health Counselor to act as a Clinical Care Manager to provide services to individuals with Psychiatric conditions and / or Substance Use Disorder.

Schedule:Monday
– Friday 9 am to 5 pm
Salary: $65,000 / year
Sign on Bonus: $1,000
The Clinical Care Manager (LICSW / LMHC) provides care coordination and clinical care management to individuals with psychiatric conditions and/or substance use disorders, many of whom receive outpatient services at Vinfen’s Behavioral Health Clinic.

The Clinical Care Manager (LICSW / LMHC) is a key member of a larger care team and works closely with Community Health Workers and a Housing Support Specialist under the supervision of a Team Leader.

The Clinical Care Manager (LICSW / LMHC) provides outreach and engagement to members within various community settings, drives the development of assessment and implementation of care plans, and collaborates with inpatient and acute care providers to develop comprehensive discharge plans to ensure continuity and coordination of care services.

Requirements
Vaccine Mandate Policy
In accordance with federal mandates, all new employees, interns, and volunteers within Vinfen’s Massachusetts programs will be required to be fully vaccinated (14 days beyond last shot of Pfizer or Moderna or 14 days beyond receiving a Johnson and Johnson vaccine), or have received the first shot in a two-dose vaccine (Pfizer or Moderna) with an appointment scheduled for the second dose.

A 2nd dose must be received within six weeks of hire and the first shot prior to New Employee Orientation or have an approved exemption for medical or religious reasons by the first day of employment with Vinfen,
Minimum of 5 years clinical and case management experienced preferred.

Must have prior experience working with people living with SMI and/or SUD.
Preference given to bi-lingual/bi-cultural applicants and those with lived experience of psychiatric conditions or substance use disorders.
Must be willing to use a personal, reliable vehicle for the role
Must be at least 21 years of age, have maintained a valid US driver’s license for at least one year, and must be able to pass a driver’s screening background check.
Must be able to become CPR and First Aid certified within two weeks of hire.

Certification trainings are offered through Vinfen.
