What You Will Do: Provide substance abuse counseling services.Provide assigned patient counseling services and case management.Systematically and continually assess the patient’s coping mechanisms and progress.Utilize appropriate teaching methods and/or referral sources.Coordinate patient’s discharge planning needs and readiness for discharge with other healthcare members.

What You Will Need: Licensure / Certification / Registration:Certification specializing in Crisis Prevention credentialed from the Crisis Prevention Institute (CPI) obtained within 1 Month (30 days) of hire date or job transfer date required.One or more of the following required:Clinical Counselor credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date.Counselor credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date.Social Worker specializing in Clinical Practice credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date.Social Worker credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date.Counselor specializing in Addiction credentialed from the Illinois Alcohol and Other Drug Abuse Professional Certification Association (IADAPCA) obtained prior to hire date or job transfer date.

Required or eligible for certification within one year.BLS Provider credentialed from the American Heart Association (AHA) preferred.

Education:High school diploma/GED with 2 years of experience, or Associate’s degree, or Technical degree required.
