Legacy Community Health Services

This is a Full-time position in Houston, TX posted August 10, 2021.

Legacy Community Health is a premium, Federally Qualified Health Center (FQHC) that provides comprehensive care to community members regardless of their ability to pay.

Our goal is to treat the entire patient while improving their overall wellness and quality of life, in addition to providing free pregnancy tests, HIV/AIDS screening.

At Legacy, we empower patients to lead better lives by promoting healthy behaviors and offering resources such as literacy classes, family planning services, and nutrition and weight management information.

Our roots began in 1981 as the Montrose Clinic, with specialization in HIV education, testing, and treatment.

Since then, the agency has expanded to 10 clinics in Houston, one in Baytown, two in Beaumont, and one in Deer Park with extensive services that include: Adult primary care, HIV/AIDS care, pediatrics, OB/GYN and maternity, dental, vision and behavioral health.

We also service students within KIPP and YES Prep schools.

Job Description The Public Health Clerk will provide administrative support to the Public Health team in their assigned region so they can better and more efficiently help patients and community members reach their health goals.

The Clerk will engage with patients by telephone to determine their needs for appointments and link them to services such as walk-in testing, PrEP, RD services, clinic appointments, and various health education sessions and classes.

The Clerk will collect appropriate information, document in the Legacy EMR, obtain needed patient information and forms, make and confirm appointments, email telehealth credentials and/or educational materials, and perform other duties that support Public Health staff and programs.

The Public Health Clerk provides an essential function as the first link to patients on behalf of the Public Health team.

Essential Functions * Manage the Public Health appointment request lists by contacting requesters daily, making appointments according to current scheduling guidelines, and documenting dispensations.* Complete internal referral orders for Public Health services in their region daily, including contacting patients and making appointments according to current scheduling guidelines.* Send credentials to patients for Telehealth visits.* Collect, scan, and/or enter appropriate patient documentation and Intake Forms into the appropriate database and into the LEGACY Electronic Medical Records System (EMR).* Perform other outgoing telephone calls for patient outreach such as follow-up on no-show appointments and appointment reminders.* Maintain an updated Outlook calendar and Inbox.* Attend required meetings for their region, Public Health, and the agency.* Demonstrate critical thinking skills to support problem solving to minimize lagging tasks and ensure appropriate client relationships.* Follow the Legacy guidelines related to the Health Insurance Portability and Accountability Act (HIPAA), designed to prevent or detect unauthorized disclosure of Protected Health Information (PHI).* Use oral and written communication skills to effectively convey ideas in a clear, positive manner that is consistent with the LEGACY Mission.* Maintain established Legacy policies, procedures, objectives, quality assurance, safety, environmental and infection control.* Implement job responsibilities in a manner that is consistent with the LEGACY Mission and Code of Ethics and supportive of Legacy’s cultural diversity objectives.* Perform other related duties as required Education & Training Requirements * High School Diploma or equivalent Work Experience * One (1) year of customer service experience, preferably in healthcare* Pleasant, professional, and articulate telephone voice* Ability to multi-task and handle many incoming calls or patients at once* Efficient and accurate data entry skills* Proficiency in basic computer skills (e.g., Microsoft Office, Outlook) Benefits * 9 Holiday + 1 Floating Holiday* PTO* 403b Retirement Plan* Medical / Vision / Dental (if eligible)
