The Direct Support Professional provides self-care training and assistance in daily living to individuals with developmental disabilities attending DayHab or Senior program.

SUMMARY: Assist individuals with developmental disabilities with skill acquisition, retention, or improvement in self-help, socialization, and adaptive skills. Assists in the development and attainment of life goals. Provides support and services to individuals to attain independence, productivity, and integration.

RESPONSIBILITIES: Provides individualized care as outlined in the individual’s Life Plan/ IPOP. Assists individuals in the primary activities of daily living (toileting, feeding, cooking, and cleaning) as outlined in the individual’s Life Plan/ IPOP to promote independence. Maintains files and produces service notes and documentation on assigned caseload. Documents and submits reports to the program as outlined in the assigned department.

QUALIFICATIONS: High School diploma or GED preferred but not needed. Valid NYS driver’s license with a driving record that is in accordance with the requirements of our Liability/Automotive Insurance carrier.
