To be an advocate for the individual and ensure all health and safety needs are met with dignity.Position Duties and Responsibilities1. Contribute to the evaluation of the individual’s progress and provide input to the team with regard to the development of appropriate plans to achieve specific ISP outcomes. 2. Complete written and verbal reports/ documentation related to daily progress. 3. Act as a positive role model to individuals served.4. Assist with hygiene, grooming, assist with medication delivery,housekeeping, laundry, cooking and any other areas identified within the ISP ( LSC ONLY ) 5. Provide transportation (using personal vehicle) the individual served to access the community. 6. Identify social, economical, recreational, and educational services in the community that will meet the individual’s needs.7. Complete all required trainings and work within all HeartWell Policies & Procedures.Position RequirementsMinimum – High school diploma or equivalentCurrent and Valid Driver’s LicenseSafe insured/ registered vehicle
