Job Description:The Supported Housing Program provides housing for chronically homeless single adults who are eligible for SSI or SSD, and suffer from Serious and Persistent Mental Illness (SPMI) or are diagnosed as Mentally Ill and Chemically Addicted (MICA).
It also provides housing for SPMI individuals who reside in OMH psychiatric centers, Article 28 inpatient hospitals, residential treatment programs and adult homes as well as housing for individuals who are SPMI and have been residents of either OMH Psychiatric Centers or OMH operated residential programs for one year or longer.We are looking for a full-time Credentialed Drug and Alcohol Counselor to join our Supported Housing Team!
The counselor will maintain a caseload of clients within the housing program and work cohesively with surrounding substance abuse programs, social workers, medical and social service entities, directors, and colleagues alike.In this position, the ideal candidate will:Perform initial assessments and intakes with clients residing in apartment program.Maintain caseload and all necessary documentation.Create or continue individualized treatment plans with clients.Create or continue relapse prevention plans with clients.Crisis intervention when necessary.Responsible for follow-up with client doctor and treatment team(s).Travel to other sites from Sullivan to Orange Counties when necessary.Meet with the Director and Housing Coordinator regularly.Experience:Substance Abuse: 1 year CASAC experience required.
3 years (Preferred)Mental Health: 1 year required.
3 years (Preferred)Qualifications:One year of experience working in substance abuse field (three years preferred.)CASAC Certification required.Excellent interpersonal and engagement skills.Education:Alcohol and Substance Abuse Credential REQUIRED.
